Therese Chan Tack, DO, MPH
Associate Professor
Dr. Therese Chan Tack is an Associate Professor of Clinical Medicine. She is a dual-board certified obesity medicine specialist and internal medicine clinician.
She led COVID-19 telehealth expansion across UCSF and its affiliates as the Physician Lead of Telehealth Programs for UCSF Health. She authored the Telehealth Communication Tips resource for providers, which has garnered 1000+ views and is used in residency and student training. She has helped launch initiatives like but not limited to Patient Entered Vitals, On-demand Video Interpreting, Integrated Video Visits, and Remote Patient Monitoring. In addition, she uses her LEAN training to consult with departments to improve their video visit workflows. She has presented work at regional and nationals conferences including departmental grand rounds, UC Tech Summit, UC Annual Telehealth Symposium, and ACP Physician Leadership program.

She previously led the Division of General Internal Medicine (DGIM)'s telehealth expansion efforts in 2019, having launched and developed video visits at UC Primary Care since 2017.
She led quality improvement (QI) projects in outpatient colorectal cancer screening and hypertension management. The effort led to regional and national level poster awards at SGIM and ACP. She is a current member of the DGIM Quality Improvement committee. Projects include EHR-inbasket optimization and type 2 diabetes visit access and patient engagement
Prior to joining UCSF in 2016, Dr. Chan Tack practiced primary care, urgent care, and hospital medicine with Santa Clara Valley Medical Center, a Stanford University affiliate. Her clinical leadership was recognized with the residency preceptor of the year award.
She completed categorical residency training at Loma Linda University Medical Center and served as Chief Resident. She earned a Master of Public Health degree at UC Berkeley with a focus in Health Policy and Management.
