About Andreana’s Role/Ongoing Projects
Andreana is regularly involved with process improvement projects for different programs at SFVAHCS. She finds it exciting to work in a team with different disciplines to make improvements to support patient care. The process improvement projects are important to optimize processes for staff and patient care at SFVAHCS by better utilizing resources and unburdening clinical staff when possible.

Her favorite aspect of her position is working with the people in the Division and developing meaningful, working relationships at the program level, service level, and executive level.

What is a professional goal you would like to accomplish by the end of this year?
Andreana's professional goal for this year is to be fully staffed.

Andreana studied abroad in Zimbabwe!

Andreana enjoys being outside, gardening, and camping.

Andreana admires her parents for their perspectives and wise thoughts about life.

What are some causes you care about?
Andreana cares about environmental cause and people underserved.

Andreana would be an Ethnobotanist!

About Irina’s Role/Ongoing Projects
They were selected as a site for the GUIDE model and started on July 1, 2024. GUIDE is a model that provides more support for caregivers and patients with dementia. Irina is excited to share more dementia resources from GUIDE with caregivers and patients in her program. GUIDE will definitely help homebound, dementia patients and caregivers, especially those that are burned out and need support. The team will be able to offer more services and provide more patient-centered care. They will also develop a care plan for each patient, monitoring them on a monthly basis, and provide patient and caregiver outreach.

Her favorite aspect of her position is being able to bring primary care to patients and to meet them where they are. It's very exciting, and it's very rewarding, and her favorite part is to hear how thankful patients are for the services provided.

What is a professional goal you would like to accomplish by the end of this year?
Irina's professional goal for this year is to grow the practice, increase the patient census, and eventually hire more staff
