Congratulations to Dr. Sudore and PREPARE Team on Gift

Congratulations to Dr. Rebecca Sudore and the PREPARE Team on a newly-awarded $28,000 gift from The Y.C. Ho/Helen & Michael Chiang Foundation to update of the PREPARE For Your Care Advance Care Planning Toolkit and Pamphlet to support Clinicians, Social workers, and Case Managers in Initiating ACP Conversations with English-, Spanish-, and Chinese-speaking Older Adults.  This funding will allow PREPARE to create culturally adapted Spanish and Chinese versions of the PREPARE ACP Toolkit and Pamphlet and share these free resources with health systems, community organizations, and communities across the US.  In the future work over subsequent years, PREPARE will also track the usage and downloads data from the PREPARE website to help inform and guide their research with a greater impact!
