TK Abraha

Welcome Teklehaimanot (TK) Abraha, DNP, FNP, Nurse Practitioner, to the Division!

Please join us in welcoming Teklehaimanot (TK) Abraha, DNP, FNP, Age-Friendly Emergency Department (AFED) Nurse Practitioner, to the UCSF Division of Geriatrics!

The UCSF Emergency Department, in partnership with the Division of Geriatrics, operates an Age-Friendly Emergency Department Program (AFED). AFED’s mission is to ensure a patient-valued older adult emergency department experience through high-quality resource optimization, community relationships, diversity of engaged health professionals, and respect for the patient's desire to attain their desired level of independence and function.

TK started this past Monday morning and will work closely with a multi-disciplinary team of pharmacists, ED providers, geriatricians, social workers, community-based providers, and other interdisciplinary professionals and services to create an optimal discharge or ongoing care plan for our patients.

TK earned his Bachelor of Science in Nursing from the University of South Alabama, holds a Master of Science in Nursing-Family Nurse Practitioner, and is a Doctor of Nursing Practice from South College. Most recently, TK worked as a Nurse Practitioner in multiple post-acute care facilities in the Bay Area. TK was also the lead clinician for Carbon Urgent Care at Berkeley, CA. His other work experience includes:

•          Kennestone Hospital, Registered Nurse

•          Memorial Hermann Hospital, Registered Nurse

•          Summit Alta Bates Medical Center, Registered Nurse

•          Kaiser Permanente, Registered Nurse

•          TELECARE CORP, Rehab Activity Leader (Activity Therapist)
