BACKGROUND
There is a fine line between child discipline and maltreatment. Perceptions of child discipline in three tribal groups living in an informal community in Kenya are reported.
METHODS
Child disciplinary practices in the Kibera community of Nairobi were surveyed among three tribal groups (the Luo, Luyha and Nubi) to determine perceptions and modes of discipline and what was perceived as excessive.
RESULTS
A random sample of 538 adults with children <18 years; 73% were mothers and 20% fathers. Overall, 98% had ever disciplined a child; 376 (70%) understood discipline to mean showing love and praising children when they do good but punishing wrong-doing, and 108 (20%) understood it to mean beating children whenever they do wrong. The majority said children should be disciplined by the mother (62%), father (47%) or whoever is around at the time of the offence (57%). It was deemed acceptable that teachers discipline children by 11% of the Luos, 23% of the Luhya and 34% of the Nubi. The most commonly used method was caning, used by 520 (97%), then pinching (40%), slapping (33%), pulling ears (20%) and denying food (19%). Sixty-three per cent of parents recognized that excessive punishment could cause fainting, and 64% that it could cause death. Factors reported as contributing to excessive discipline were domestic tension, poverty, stress and 'difficult children'.
CONCLUSION
There were consistent patterns of child discipline in three tribal groups living in Nairobi. There was a high prevalence of excessive discipline in the form of physical abuse, and clinicians should be aware of the signs of excessive discipline and be prepared to act in the interests of the child's welfare.

Millions of people are exposed to arsenic in drinking water, which at high concentrations is known to cause lung cancer in humans. At lower concentrations, the risks are unknown. We enrolled 196 lung cancer cases and 359 controls matched on age and gender from western Nevada and Kings County, California in 2002-2005. After adjusting for age, sex, education, smoking and occupational exposures, odds ratios for arsenic concentrations ≥85 µg/L (median = 110 µg/L, mean = 173 µg/L, maximum = 1,460 µg/L) more than 40 years before enrollment were 1.39 (95% CI = 0.55-3.53) in all subjects and 1.61 (95% CI = 0.59-4.38) in smokers. Although odds ratios were greater than 1.0, these increases may have been due to chance given the small number of subjects exposed more than 40 years before enrollment. This study, designed before research in Chile suggested arsenic-related cancer latencies of 40 years or more, illustrates the enormous sample sizes needed to identify arsenic-related health effects in low-exposure countries with mobile populations like the U.S. Nonetheless, our findings suggest that concentrations near 100 µg/L are not associated with markedly high relative risks.

Current European atrial fibrillation (AF) guidelines have assigned a strong recommendation for the initiation of antithrombotic therapy to prevent thromboembolism in all but those AF patients at low risk (or with contraindications). Furthermore, the selection of antithrombotic therapy is based on the absolute risks of thromboembolism and bleeding, and the relative risk and benefit for a given patient. By their very mechanism of action, antithrombotic agents used for stroke prevention in AF will potentially increase the risk of bleeding events. Moreover, the introduction of novel oral anticoagulation agents have introduced new, hitherto ill-defined, deficiencies in the authors' knowledge with respect to anticoagulation monitoring, availability of direct antidotes, drug-drug interactions and the ability to appropriately control and reverse their actions if bleeding events occur. The authors present a comprehensive review on all aspects of bleeding related to currently licensed antithrombotic agents used for stroke prevention in patients with AF.
