Smart Referrals and eConsults are two important ways in which the Department of Medicine is working to reduce health care costs and improve the patient experience at UCSF.
Smart Referrals, headed by Dr. Nathaniel Gleason and Dr Ralph Gonzales, enables primary care physicians to quickly assemble essential patient information in order to optimize new patient consultations. Developed in collaboration with primary care and each subspecialty, this tool conveys information often missing in traditional referral systems, including the diagnostic evaluation completed to date, and the referring provider's expectations for co-management. e-Consults provide a mechanism for primary care physicians to receive a subspecialty consultation within 72 hours for patients that do not require an in-person evaluation. This allows rapid access to subspecialist expertise for appropriate questions, and improves access to subspecialty clinic appointments by addressing demand.
