    Project Title: A Multi-Center Trial of Academic Hospitalists

    The major goal of this project, the first multicenter examination of the hospitalist model, is to determine the effects of a hospitalist system upon patient outcomes and satisfaction at 6 academic medical centers. Dr. Auerbach has been site PI for this project since 2001, and has overseen all aspects of work at UCSF including enrollment of patients, training and management of research assistants, as well as collection of administrative data, analysis of data and preparation of manuscripts resulting from this work.
     
    Project Title: Medical-Surgical Comanagement: Evaluating a new paradigm

    Project Goals: The major goals of this research and training grant are to determine the effects of closer medical care by internists - 'comanagement' upon outcomes and satisfaction of patients undergoing orthopedic or neurosurgical procedures. This grant will also allow the PI to gain further training in statistical methods and organizational theory.
     
    Project Title: Novel Biomarkers of Perioperative Cardiac Risk

    Proposal Number # 0355003Y (AHA Western states affiliates) 25% Effort (Overlap with KO8), 5% salary support $50,000 annual direct costs
     
    Project Goals: The major goals of this grant are to 1) Assemble a biological specimen bank and clinical database of patients undergoing major noncardiac surgery at Moffitt-Long Hospital, 2) Determine whether high-sensitivity C-reactive protein is associated with perioperative myocardial infarctions, and 3) Provide pilot data for future extramural funding efforts. Funds from this grant mechanism will be used for material costs (i.e. lab costs) only.

    Project Goals: The major goals of this grant are two-fold: 1) to provide risk adjusted process-adherence and outcome data which will be used to benchmark service, floor, and physician performance, and assess quality improvement efforts, and 2) to understand patient, physician, and service-level predictors of adverse outcomes following surgery. 1,200 patients undergoing major non-cardiac surgery will be enrolled in this study between July 1 2004 and June 30, 2006.

    Project Title: Impact of inpatient care organizations on quality and outcomes

    Project Goals: The major goals of this grant are to fund analysis of data collected from more than 100 hospitals which use a private data management company (CareScience, Inc) to manage their inpatient information systems. The major goal of the study is to determine whether physicians who are physically located in the hospital (such as intensivists and hospitalists) provide higher quality care, and whether interactions between the two forms of specialists provides additional advantages or disadvantages in terms of costs, length of stay, mortality, readmissions, and use of appropriate therapy (i.e. aspirin and beta-blockade for acute transmural MI).
     
    Project Title: Biomarkers of surgical outcomes in elderly patients

    Project Goals: The major goals of this grant are to extend the preliminary study .Novel Biomarkers of Perioperative Risk. currently being supported by funds from the American Heart Association, Hellman Award, and Dr. Auerbach.s K award. This 5-year study will seek to understand whether preoperative measurement of several biomarkers (High-sensitivity C-reactive protein, interleukin-6, myeloperoxidase, fibrinogen, and d-dimer) and add to current methods for prediction of postoperative complications such as delirium, respiratory failure, myocardial infarction, and return to full function.
     
    Project Title: Improving care for Older Americans

    Project Goals: The major goals of this program project grant are to evaluate the impact of 4 specific elements of inpatient care on the outcomes of elders hospitalized at 6 academic medical centers: 1) Experience and expertise of the attending physician, 2) availability of nursing, specifically nurse-staffing ratios and nurse skill mix, 3) use of pharmacists to coordinate discharge medications and detect medication errors, and 4) availability of palliative care services. Dr. Auerbach will be the UCSF Site PI for the overall data and administrative cores, as well as the UCSF project PI for research projects 1 and 2 listed above.
