The CHF Hospitalist Service is responsible for providing the inpatient care for patients with complex heart failure, heart transplant, and pulmonary hypertension (for more details about the UCSF Division of Cardiology Heart Failure Evaluation and Treatment Center).
Beginning in July 2007, the CHF Hospitalist Service will be staffed with a unique hybrid model involving collaborative care provided by both hospitalists and cardiology faculty specializing in heart failure. The hospitalists on the CHF service are physicians trained in Internal Medicine who manage the day-to-day care of the patients while the cardiology physicians provide necessary specialized care.
The service is dedicated to providing the highest quality and safest care to a complex group of patients while hospitalized. The inpatient care will involve intensive coordination among physicians, nurses, pharmacists, therapists, and care coordination. The service will ensure safe and appropriate transitions in care in and out of the hospital.
