ACE Unit - Acute Care for Elders

The Acute Care for Elders Unit at the San Francisco General Hospital cares for hospitalized adults over the age of 65 in a specialized unit focused on preserving physical and cognitive function. Care is patient-centered and delivered by a multidisciplinary team with special training in preventing, recognizing, and treating syndromes common to the hospitalized older adult. The unit serves as a center of expertise in Geriatrics for the entire hospital.

Housecalls Program

Housecalls provides comprehensive medical services to homebound elders in San Francisco while teaching medical students and residents how to do home visits. The program provides care to patients who are typically in their late eighties who have multiple medical problems and who have not had a medical visit in months or even years. Lakeside Senior Medical Center

Lakeside Senior Medical Center provides quality health care for older adults with a multidisciplinary approach to care. Clinical services include fulltime medical nursing, and social work providers who are trained in special needs of older patients. Podiatry, psychology and laboratory services are provided on site; physical therapy and radiology services are available nearby.

Over 60 Health Center

Over 60 Health Center in Berkeley is a community-based clinic (part of the Lifelong Medical Care network) providing interdisciplinary care to elders in Alameda County. Lifelong Medical Care maintains a clinical and educational partnership with UCSF. The Over 60 Health Center was founded in 1976 as a grass-roots clinic for senior citizens. The clinic provides on-site medical, podiatry, optometry, psychotherapy, laboratory, and social services. Over 60 is one of the few remaining primary care clinics whose providers also care for their own patients if an individual is admitted to Alta Bates Medical Center, or to several local nursing homes.
